Advance care planning (ACP) is a process of discussion between an individual and their care provider. ACP is different to care planning which is an everyday tool used by many workers in the social care sector in all types of situations to ensure that the needs of individuals are met on a daily basis.

This module provides elearning sessions which aim to help you understand the principles and purpose of ACP, its benefits, and limitations and how to carry out ACP in practice.
