- e-ELCA 02: Assessment in end-of-life care

This module provides resources that will help you to understand the principles, purpose, and boundaries of effective assessment in end-of-life care.
There are sessions which:
• introduce you to the principles of assessment in the context of end-of-life care
• describe the assessment of each domain separately
• outline the assessment process, from the first meeting with the person, to identifying their priorities, documenting and communicating this information, and follow-up assessments
• discuss how assessment may be conducted in more challenging circumstances, for instance, assessing through proxies, assessing those with fluctuating mental capacity, assessing urgent situations with limited information and assessing the dying phase and what to do after death
• describe the assessment and approach to support for the family and caregivers, both while the patient is still alive and in bereavement
The resources in the e-ELCA programme discuss sensitive topics including death, dying, the different stages of end-of-life and bereavement, which may be distressing to some learners.
