This e-learning programme is intended to improve healthcare practitioner’s confidence to support patients with long term or chronic health conditions, via brief conversations in routine consultations. It does not cover work-based health initiatives, but instead focuses on the relationship between health professionals and patients. Research shows that multidisciplinary teams are highly effective in helping people with long term conditions return to or stay in work.
On completion of the Introduction and Self-care sessions a certificate of completion will be generated. You will need to gain a pass rate of at least 80% to complete the Introduction session. A certificate of completion will also be generated for each of the Supplementary Information and Signposting sessions. Each session lasts approximately 30-40 mins. - Created:
