This learning path includes a number of sessions that will support professionals in caring for people with heart failure.

There are a number of foci for this learning path: trajectory of illness and planning for the future, discussions around specific interventions, medical management of heart failure, and symptom control (particularly of breathlessness).

The learning path is designed for health care professionals involved in the care of patients with end stage heart failure.
