The aim of this session is to enable you to develop your knowledge and understanding of prehabilitation, rehabilitation and Personalised Care and Support Planning (PCSP) for people living with cancer, and to allow you to apply the theoretical underpinning to your practice. We will also explore concepts underpinning the personalised care approach including supported self-management and principles of behaviour change.
