- Description:
- Discharge planning should start very early in a patient's in-hospital journey. This session is aimed at familiarising you with the therapy support available on the ward and the appropriate time for referral. It also examines the importance of liaising with social services to ensure the smooth transfer of patients from secondary care back to primary care. It prompts you to think about your role in the multidisciplinary team, including how to interact with your colleagues with integrity whilst promoting patient safety.
